7/4/2023 EnchantaVaughn Approved Behavior: When QMHP came inside the home, client was seen in a friendly mood, presenting a happy affect. Client was observed finishing cleaning up and preparing for her session. Client asked for permission to go outside after her therapy was done. Her mom agreed and gave her space to talk to her counselor for a little bit. Client communicated high level of energy and initiative to review her objectives with QMHP. Client did not show any signs of distress. Client was polite and courteous and sure to listen and follow directions. Client presented a good attitude throughout the session. Client was supported for staying involved in the session and showing excellent manners. Behavior: Client was observed cooperating with parent and VACCD staff and displaying good attitude. Client stated that she was feeling goid. Client expressed that she finished all of her work. Client expressed that she helped her mom around the house, did all her chores, and finished her homework. Client began completing her therapy assignment without any further distractions. Client maintained appropriate behavior and was applauded for her cooperative manners towards all authority. As the client has increased her verbalizations of emotions, she focused onto her feelings that were motivating and uplifting. Client continued to express positive feelings and demonstrate positive behavior in the home. Intervention: QMHP observed clientâs behaviors as seemingly calm and controlled as demonstrated by her ability to release her energy appropriately in a social environment without verbal instructions. QMHP reiterated to client the benefits of following the rules and of the privileges she can receive when she does so. QMHP provided simple non-verbal praises as client was observed displaying positive behaviors. QMHP applauded client for her commitment and efforts with doing her best and having a great week so far. QMHP reinforced clientâs positive behaviors by modeling positive behaviors. Intervention: QMHP offered simple praises to client for exhibiting the appropriate behaviors during the session. QMHP utilized cognitive skills to give the client a chance to openly and honestly discuss her feelings about her current state. QMHP continued to establish a positive supportive relationship with the client. QMHP will review and give feedback regarding ADHD and its causes and effect to well-being at home, as well as, in the community. QMHP reviewed clientâs ISP objectives with her in efforts to reinforce her commitment to achieving them on a daily basis. QMHP aided client in recognizing her target behaviors in efforts to reinforce the importance of complying with social norms. Response: Client responded well to the support provided by QMHP. Client promised to treat others with respect the same way she wants to be treated. Client seemed to be coping with her stressors and attempting to correct her mistakes as much as possible. Client promised to respect others personal space and boundaries. Client asked appropriate questions related to her behaviors at home. Client displayed appropriate behavior in therapeutic area. Client displayed good interaction with QMHP. Client was polite and courteous to everyone in the home. Response: Client demonstrates leadership skills by helping others in the home. Client displayed independence with completing her work in the session. Client responded well to feedback and seemed to be adapting well to the therapeutic environment. Client displayed high ambition and was very productive, despite distractions. Client stated she would avoid distractions and stay on task with no problem. Client stated she would help others when she can. Client stated she would treat others fairly. Client stated she would respect adult figures and do what she is told. Client verbalized positive statements about in-home services.
